A Focus on Better Health Earns Incentive
During open enrollment members and their covered spouses may earn incentives to lower insurance premiums. These wellness programs are intended to minimize your health risks and reduce your costs.

The wellness incentive has two parts:
1) Certify tobacco-free status or enroll in a coaching program as an alternative standard.
If you are a tobacco user and ready to quit, enrolling in the Medical Mutual’s Quitline tobacco cessation program by May 1, 2017.
Enrolling in the Quitline will earn you the tobacco-free incentive on July 1.
To receive the family coverage premium reduction, BOTH the employee and covered spouse must either certify as being tobacco-free, or complete the tobacco cessation program.
2) Complete an annual physical and preventive care activities with your Physician and submit the required paperwork. Complete this process by May 1, 2017.
Physicals completed after May 1, 2016 qualify you for the incentive. You will need to print the form and have your physician sign it. You will need to submit the form.
To receive the family coverage discount, BOTH the employee and covered spouse must complete the physical and submit the physician form by May 1, 2017.
