I acknowledge that I have read and that I understand and agree to adhere to the Catholic Diocese of Cleveland Electronic Communication Policy.  I acknowledge that all communications created, sent, received and/or accessed using the Electronic Communication System are Diocese property.  In signing this form and by my continued use of the Electronic Communication System, I consent to the Diocese monitoring, accessing and disclosing all electronic communications and electronic communication systems, including e-mail, instant messaging, voicemail, personal digital assistant and Internet messages and data, at any time, that are created or accessed using Diocese-provided equipment.

I understand that failure to adhere to this Policy can result in disciplinary action, up to and including dismissal from employment with the Diocese.
