There are several forms available to be used by a petitioner, the person who wishes to apply for a declaration of invalidity. The choice of form is made by the procurator after a discussion with the petitioner. PRINT YOUR OWN FORM A few of the most popular forms are available by clicking on the highlighted words below. FORMAL PETITION -Revised June 2008)- This will print the most current version of the Formal Petition (T101). If you wish, you may print these pages on normal 81/2 X 11 stationery. If you would like to print this onto the same paper used by the Tribunal, use 11x17 paper in your copy machine Procurators: Please remember to sign on page one AND page two of the Libellus. The selection depends upon whether or not the marriage you are having the Tribunal evaluate was a VALIDATION. use the normal PETITIONER's QUESTIONNAIRE -Revised June 2008 if the marriage is NOT a validation. This is the two-page questionnaire that supplements the petitioner form. When printing it is suggested that you print side one. Then feed the paper in to print page two on the reverse side. If the marriage was a validation, use the PETITIONER QUESTIONNAIRE (MARRIAGE WITH VALIDATION) NEW June 2008!
