Forms for Procurators

There are several forms available to be used by a petitioner, the person who wishes to apply for a declaration of invalidity. The choice of form is made by the procurator after a discussion with the petitioner.

 

1. FORMAL PETITION -(Revised June 2008)     Download Formal Petition.pdf  updated 1-28-2010
This will print the most current version of the Formal Petition (T101). If you wish, you may print these pages on normal 81/2 X 11 stationery. If you would like to print this onto the same paper used by the Tribunal, use 11x17 paper in your copy machine Procurators: Please remember to sign on page one AND page two of the Libellus.

 

2. PETITIONER'S QUESTIONNAIRE (Revised June 2008)
NOW THERE ARE TWO different PETITIONER QUESTIONNAIREs to select from. The selection depends upon whether or not the marriage you are having the Tribunal evaluate was a VALIDATION.  A Validation occur when either party is Catholic and are first married civilly, then at a later date,  present themselves to the Church for a Catholic ceremony.

If the marriage is NOT a validation. This is the two-page questionnaire that supplements the petitioner form. When printing it is suggested that you print side one. Then feed the paper in to print page two on the reverse side.

This new form is REQUIRED of a petitioner who is unable to obtain complete address information for the respondent.

First Step

(a.k.a. QuickStart)

...  is a simplified manner to begin your petition for a Declaration of Invalidity (a.k.a. Annulment). 

The First Step program does not shorten the process of hearing a case.  

It is intended to simplify the submission of a petition. 

The process of hearing a case is defined by Canon Law and remains the same for all cases.

You are invited to attend:
Session 1 at any location then schedule an appointment at any Session 2 that fits your schedule.*
